Any telephone calls/answerphone messages received after 4.00pm will not be actioned until the following working day.Patients can be referred by their GP, community nurse or any other health care professional. You can contact the Tissue Viability Service by telephone, fax or email . Messages can be left on answerphones.

How and when to refer to Tissue Viability:- 

Refer to the Tissue Viability team for complex wounds (including cat 3 & 4 pressure ulcers)  or those which fail to progress at the 6 week point. Please complete a complex wound referral form: – Complex_wound_Leg_Ulcer_referral_form and send to Tissue Viability via email or fax. Preferably with colour photos. TV recommend that you send test document first to ensure that they have the correct email address and for patient confidentiality.
