POCT in the Community – The Strategic Case
“Unity is strength… when there is teamwork and collaboration, wonderful things can be achieved” (Mattie Stepanek)
The current practice of delivering Point of Care Testing (POCT) both within the primary and secondary care setting has evolved, rather than been designed with the patient in mind. It can be fragmented, particularly within primary care. It is clear that, to take advantage of developments in technology to support patients with long-term conditions (LTCs), the current model needs to be changed.
The following pages give an overview of POCT in the community. They discuss a new approach to partnerships between Clinical Commissioning Groups (CCGs), their respective Trusts and third parties such as social services; and how this can improve the patient pathway, experience & clinical outcome and also introduce savings to both the CCGs & Trusts.
The patient model resulting from a collaboration.
